Send a current, individual head & sholders picture for the yearbook. We then will not have to use Alfred E. Neuman. Use the browse button to find your picture file on your computer.

And then click Send Picture to upload it. Reset the above if you screwed it up

• Who did you marry and when?
• Where have you lived?
• How many children and grandchildren do you have?
• What accomplishments or hobbies would you like to share with your classmates?
• What other highlights would you like to share?

When the form is correct click on Submit to send it to the Yearbook committee.
Use the Reset Button to clear the form and start over.

If you want, you could type up your bio information and then paste it in when you fill out the form.

Or (and this is the worst case because I have to retype everything) fill it out,
print it (Browses menu File, Print )and send it with included picture by snail mail to:
